Death Certificate

Death certificates from 1949 to the present are available through the Department of Health and Mental Hygiene. For earlier records, contact the Municipal Archives at 311 or 212-NEW-YORK if outside of New York City. Their address is 31 Chambers Street, Room 103, New York, NY 10007. You may also access the Municipal Archives' Web site at www.nyc.gov/doris. The Office of Vital Records has two ways that you can apply for a copy of a death certificate:

Via Mail

Send us a Death Certificate Application or a letter stating the following information:

     * Full name as listed on the certificate
     * Gender
     * Date of death
     * Place of death
     * Borough in which the death occurred
     * Decedent's SSN
     * Decedent's mother's maiden name
     * Decedent's father's name
     * Spouse's name
     * Age at time of death
     * Purpose for which you are going to use this certificate
     * Your mailing address
     * Your relationship to the deceased

Along with application or your letter, please include:

* Check or money order payable to the NYC Department of Health and Mental Hygiene; the cost is $15 per certificate
* Copy of your signed, valid photo identification
* Stamped, self-addressed envelope

Please allow two to four weeks to receive your certificate(s)

If you are applying for a certificate on behalf of someone else, you must provide us with an original, notarized letter signed by that person authorizing release of their certificate to you. You must also bring with you that person's photo ID as well as your own.

What information you should be prepared to provide:

     * Full name as listed on the certificate
     * Gender
     * Date of death
     * Place of death
     * Borough in which the death occurred
     * Purpose for which you are going to use this certificate
     * Your relationship to the deceased
     * Your mailing address
     * Signed, valid photo identification of yourself

The cost is $15 for each certified copy. You may pay by using cash, check or a money order. Please bring a pen.

Note that death certificates are not issued the same day. Certificates are mailed within one to two weeks or, in cases of emergency, they may be picked up.
